Sane is a doctor. After transmigrating to another world overrun by the bizarre and uncanny, it was only natural that he became a healer. But something went wrong: his soul transmigrated into the body of a chainsaw-wielding killer.

In this new world, demons, evil spirits, and all manner of sinister beings run rampant. But demons cannot travel directly from Hell to the human world. They can only possess people to meet their murder quota. The possessed then become proficient in both physical combat and magic, moving like zombies and casting spells like Merlin himself. Their fighting power is top-tier.
To exorcise them, humanity has tried everything, with the methods falling into three broad schools.
First: the orthodox academy school.
Priests and nuns from the Church hold a Bible in one hand and a cross in the other, shouting Jehovah's name as they borrow power from God and try to drown demons in a flood of spittle.
Unfortunately, God is almost never around, and the Church is always embroiled in infighting. By the time their skills are ready again, they've already been kicked eight hundred meters away.
Second: the folk occult school.
For those who aren't religious and can't borrow power from God, there are all sorts of unconventional methods to try: borrowing power from demons in exchange for the souls of friends and family, sealing one's own soul inside an item so coworkers can use it to exorcise demons, or refining coworkers into magical artifacts so you can exorcise demons yourself.
They work pretty well, but they cost you and your parents and closest friends their lives. Orphans aren't allowed to join this school. Join it, and you'll end up an orphan.
At this point, someone asks: Exorcist, exorcist, is there a way to exorcise demons that doesn't cost money or require family?
Sane: There is, there is.
It's time to give the Western world a taste of physical exorcism.
Sane holds a scalpel in his left hand and a chainsaw in his right. First, he uses his right hand to take apart the possessed person, so they can't kick him into the eighteenth level of Hell during the operation. Then, after the exorcism, he uses his left hand to stitch the patient back together and bring them back to life.
One hand kills, the other saves. In the end, all the demons are driven away, and everyone gets a happy ending. Wonderful!
After the surgery, Sane looks up in awe, his hands covered in blood and blood dripping from his monocle, only to see everyone staring at him in terror.
The patient is terrified. His coworkers are terrified. The Church is terrified. Even the demon he sent down to Hell with his roaring chainsaw is terrified.
Sane: ?
Sane has no idea what's going on, but two days later, he still has to go collect his award for being the greatest exorcist in the field.
[Saving the world with medicine, healing the sick and killing people. To Sane]
