But the villain, fallen into the dirt and covered in wounds, coughed up blood as he gave me a smile so twisted it was almost sickening. He gripped my neck, brimming with excitement, and said, "Dear little sister, I've finally found you again!"

What was that "again" supposed to mean?
So he'd known it was me all those times and had just been waiting for me!
