From the loopholes in the CTQ review process to the missing configuration of CCD visual inspection, from the MES strong lock of OQC not being enabled to the lack of double signatures for excipient replacement, Yang Xiaohui dug down layer by layer and found that the root cause of this accident was never the error of a single operator, but a whole set of systems that allowed errors to penetrate all lines of defense.
