I don't understand why the author would write Yotsumi Tsumuko's name as Shizumi Tsumuko. It doesn't matter if it looks awkward if written like this. The title is still Miko, and other people's title is still Miko. Then why do you write Shizumi Tsumuko? Just write Yotsumi Tsumuko.
