Introduction
Based on the service practice of "integrated medical care and nursing care", this book innovatively proposes a new urban elderly service model of "community health care synergy". Based on a large amount of empirical data, the book analyzes the supply transformation driving force and demand upgrading driving force of the "community health care collaboration" model. Based on the overseas experience of the U. S. PACE project, Japanese long-term care insurance, and Swedish home care, the basic ideas for developing the "community health care collaboration" model in my country's cities are explained. Based on the perspective of collaborative governance, the "community health care collaboration" model is analyzed in order to build a new urban elderly service model with "diversified subjects", "collaborative relationships" and "unified rules" to meet the elderly's demands for healthy and elderly care in an efficient and high-quality manner. He is not in good health and his children are not around him, but through the health care services provided by the community, his quality of life has been greatly improved.
